Proof of identity

Applicants for Driver Accreditation must show proof of identify (POI).

The following proof of identity needs to be presented:

  • Evidence that you hold a current full Victorian driver licence and have held a driver licence in Australia or New Zealand for a minimum of six months.
  • You must provide separate documents as primary and secondary evidence with one document containing your signature.
  • Only original or certified copies of POI documents will be accepted. Original documents will be sighted, copied and returned immediately.
  • Applying by mail (or if the person whose POI documents are required is not present when application is lodged).

Photocopies of original POI documents must be included with the application and certified by either a member of:

  • the police force
  • an accountant (Chartered Accountant (CA)
  • Certified Practicing Accountant (CPA)
  • Member of the National Institute of Accountants (MNIA)
  • bank manager
  • honorary justice (JP)
  • medical practitioner
  • dentist
  • school principal
  • veterinary surgeon
  • pharmacist
  • barrister or a solicitor.

To obtain certification you must present the original documents and a photocopy to one of the people listed above. The person authorised to certify must write on the photocopy 'I certify that this copy is a true and accurate copy of the original' and also state their name, occupation and date.

Differing name or change of name

Where there is a difference in name between the primary and secondary evidence documents, or to change a name, you will need one of the following documents:

  • Marriage certificate issued by a Registrar of Births, Deaths & Marriages in Australia
  • Divorce papers (bearing the name being reverted to)
  • Deed Poll (pre November 1986 in Victoria), or
  • Change of Name Certificate (post November 1986 in Victoria).

Proof of Identity requirements for conducting a National Police History Check

As part of the Driver Accreditation process, you will be required to provide proof of identity for the purpose of conducting a National Police History check.

All documents must be certified true copies. A certified copy means a document that has been certified as a true copy of an original by a person listed in Schedule 2 of the Statutory Declarations Regulations 1993 (Cth) – These persons include a Police Officer, Medical Practitioner, Legal Practitioner, Pharmacist and Dentist. Change of Name

If all documents provided for 100 points of ID are under the same name you will not be required to provide additional ID documentation if you provide a maiden/alias name.

If the 100 Points of ID provided are under two or more different names (e.g. birth certificate in maiden name and driver's license in married name) then further ID documents will need to be provided as evidence of a name (e.g. Change of Name or Marriage Certificate issued by a State or Territory Registry of Birth, Deaths and Marriages, or Divorce Papers issued by the Family Court).

These documents must be originals or certified true copies and do not count towards the 100 Points.

If you use a change of name document you must have provided the other names you have used in Section 1 of this form.

Minimum Identity Requirements

You must provide:

  • at least one document from either Category A or Category B, that is, you do not need to provide documents from both categories, as long as all other minimum requirements are satisfied;
  • at least one of your identity documents must contain a photograph. If you are unable to provide a listed document containing a photograph you must submit a passport style photograph of yourself certified by a person listed in Schedule 2 of the Statutory Declarations Regulations 1993 (Cth). – as detailed above;
  • the combination of documents supplied should, as a minimum equal a total of 100 points; and
  • evidence your full name and date of birth.

  • Aviation Security Identification Card/Maritime Security Identification Card
  • Public Employee Photo ID Card (Government Issued)
  • Department of Veterans' Affairs Card
  • Centrelink Pensioner Concession Card or Health Care Card
  • Current Tertiary Education Institution Photo ID
  • Reference from a medical practitioner (must have known the Applicant for a period of at least 12 months)
